- Endorsement letter from the Bureau of FIre Protection
- Photocopy of the valid of the PNP Personnel
- Copy of PSA-Authenticated Birth Certificate of both BFP personnel and dependent
- Certificate of No Marriage for PCG personnel whose siblings are applying in NU.
