- Endorsement letter from PNP Provincial Director
- Photocopy of the valid of the PNP Personnel
- Copy of PSA-Authenticated Birth Certificate of both PNP personnel and dependent
- Certificate of No Marriage for PCG personnel whose siblings are applying in NU.
