Requirements
Awarded to students from the identified Science high school who maintained a grade of 88% in all subjects from 1st to 4th quarter in Senior High School with an annual family income not exceeding Php360,000.00 per year.
- Latest tax exemption certificate or income tax return of both parents with an annual family income not exceeding P360,000 per year
- Two (2) recommendation letters from school administrators or faculty (attach a photocopy of valid ID of the recommender) addressed to Admissions Director
- Complete Grade 12 Report Card (Form 138)
- Certificate of Good Moral Character
- Original copy of PSA-Authenticated Birth Certificate
- Recent 2×2 Colored ID Picture with white background
