Qualified dependent of the AFP who is the recipient of the educational benefit grant.

• Endorsement Letter from AFPEBSO
• Copy of PSA-Authenticated Birth Certificate of both AFP personnel and dependent
• Certificate of No Marriage for PCG personnel whose siblings are applying in NU.
